This is a template for our Non-profit team to be used internally. The purpose is to minimize time filling in the numberous form a client service representative would need to process an order by filling this template out and inserting pages in Adobe Acrobat.
Some highlights on this page are the dropdown menus for the dates and the stylized javascript field used to fit longer company names to avoid confusion when filling it out.
The buttons on this page minimize the time you take to fill it out by having button to autpopulate the physical address from the first page by selecting the button.
Calculations are done via javascript code to save time in the CSR's workflow.
